A US safety regulator, investigating the cause of a blast at BP Texas oil refinery last spring, where 15 people were killed and more than 100 injured, has studied out an alarm malfunction.
The Chemical Safety and Hazard Investigation Board said high-level alarms at the plant eventually functioned, but only after the explosions had begun.
